The story continues to follow Dr. Amy Larsen, who is still struggling with memory loss from the past eight years, and this time, an unexpected visitor from her past shakes things up even more.
The episode opens with a case that blurs the line between mind and body. A young woman named Megan walks into the hospital convinced she’s pregnant, even though the tests show otherwise. She belongs to a commune that avoids modern medicine, which makes things trickier for the doctors. Megan shows all the signs of pregnancy—nausea, abdominal swelling, and emotional attachment to her “baby.” But when Dr. Amy performs an ultrasound, she finds no fetus. Instead, she discovers a large mass. Tests reveal that Megan isn’t expecting a child at all; she’s suffering from pseudocyesis, a psychological condition where a person believes they’re pregnant and even experiences physical symptoms of it.

The tumour is removed successfully, and by the end of the episode, Gina helps her reconnect with her estranged brother, Jordan. The emotional reunion gives Megan the comfort she needs as she faces the truth about her condition.
While that case takes most of the spotlight, another big moment in the episode comes with the unexpected return of Dr. Joan Ridley, played by Felicity Huffman. She has been gone for five months and makes her comeback as Amy’s former mentor. Her reappearance immediately stirs old feelings and rivalries, especially since she wants her old position back as Chief of Internal Medicine. Her presence not only challenges Amy professionally but also brings up memories of the past—particularly the night Amy chose to have dinner with Joan instead of reconciling with Michael, a decision that ultimately ended their relationship.
Joan reminds everyone why she’s a respected doctor when she swiftly diagnoses a man suffering from endless hiccups, discovering that a lipoma was the root cause. But as the episode progresses, it becomes clear that her influence extends far beyond the hospital floor.
